Longwood House Dental Care Terms of Service

Fees:
We provide dental care on a Private basis only, where patients pay a fee for each item of treatment provided, or subscribe to a dental care plan. Estimates for proposed recommended treatments will be discussed by your dentist and given in writing before any treatment is carried out.
Fees for treatment are due on the day the treatment is provided. We do not operate an account in arrears facility. In some circumstances, we reserve the

right to request payment in advance for certain treatments. Payment methods accepted are:

Cash
Cheque, with a valid Cheque Guarantee Card
Debit/Credit Card (Not American express)

Please note that unpaid accounts are routinely referred to a Debt Collection Agency or the Small Claims Court, and we reserve the right to recover all costs incurred in doing so.

Where treatment incurs a laboratory fee, 50% of the total fee is due at the appointment where impressions are taken.
In the case of dental treatment provided under insurance schemes or finance agreements, the patient remains liable for payment.
Fees for treatment where sedation is included must be settled prior to the appointment to avoid financial transactions, or signatures being required, whilst a client is still under the influence of the sedative.

Treatment estimates:
Once your treatment plan has been agreed with the dentist, we will provide written details of your plan. If this plan changes due to radiographic or clinical findings, we will inform you and discuss this with you. Treatment plans are valid for 90 days from examination.

Consent forms:
Certain treatments require completion of a consent form e.g. dental implants. We will explain the treatment, aftercare and any risk to you thoroughly prior to asking you to sign the forms. Treatment cannot proceed without the signed forms.

Emergency appointments:
We request that, should you need to see a dentist urgently, you contact Longwood House Dental Care as early as possible.When the surgery is open,the phones are answered from 9.00 am. We will respond to a message left on our answerphone as soon as possible; please ensure contact details are clear (please note the surgery is closed on Thursdays).

Late cancellation or missed appointments:
We appreciate that you have busy lives which may mean that you are not always able to attend a previously scheduled appointment. Please contact us and we will endeavour to rearrange to your convenience. However, please do appreciate that failure to attend an appointment or inadequate notice of cancellation (less than 24 hours) means that we are unable to offer the appointment to other patients and therefore means we will have to charge a cancellation fee. Missed Hygiene appointments are charged the full fee and missed dental appointments are charged dependent on the length of the appointment missed.

Personal Details:
It is very important that you give a full medical history and details of any medication you take. Should these change in anyway it is important you tell the dentist and give current information as this may affect the recommended treatment. Failure to provide any of this information could be dangerous to your health. Longwood House Dental Care cannot be held responsible for anything resulting from the failure to provide a full and accurate medical history.

Data Protection Act:
We store all patient personal details in accordance with the Data Protection Act. All clinical notes, radiographs, photographs etc remain the property of Longwood House Dental Care. Copies of notes, radiographs and photographs can be made available on request. Longwood House Dental Care reserves the right to make an administration charge for any copying.

Dental notes are commonly abbreviated and we are happy to explain these abbreviations to you.

TREATMENTS WITH OTHER CLINICIANS- AYERVAYDA CLINIC , ANTIAGEING & BEAUTY CLINIC
The clinics run independent of one another. The therapists are responsible for the services they provide. Please forward any comments, concerns or complaints to your treating clinician.

In-house promotional offers:
All offers are subject to the discretion of the treating dentist. All offers are for a limited period only.

Free Examinations:

Offers are available to new patients and not those already registered at the practice.
An examination does not include a scale and polish, x-rays or any other treatment.
This offer cannot be used in conjunction with any other offer.

Invisalign - £350 off:

Offers are available to all patients including those not registered at the practice.
Suitability for treatment will be assessed on examination.
This offer cannot be used in conjunction with any other offer.
£350 off is subject to acceptance of the Invisalign treatment (Clin-Check).

Teeth Whitening - £100 off:

Offers are available to all patients including those not registered at the practice.
All patients will require an examination to check for suitability of treatment. This needs to take place before the whitening treatment is booked.
This offer cannot be used in conjunction with any other offer.
This offer is available subject to availability.
